According to The West Stats, in world 4, only New Waterloo and Red Baron has it. Thats quite rare considering not many new town currently being built in W4.

I don't really know what Pilgrim Gown is but his shoes can be found in chaseville

There's a male Pilgrim outfit and a womens. The women's items seems to be quite hard to find.

true, one more weird thing is, the gown give 2 str and shirt give 2 dex. it supposed to be vice versa

The town I'm in (Abilene) has it, and I have it in my w1 town too.

That depends. Perhaps the fairer sex being a bit weaker, they would need more strength from clothes stats and males being more clumsy might need additional dex stats
